Dental insurance is available to all TDCJ employees. If enrolled, employees may also enroll their eligible dependents. Premiums are based upon the option selected.
Provides specific discounted treatment fees through contracted dentists. A primary care dentist must be selected from a list of approved providers.
Premium
Premiums vary according to the level selected.
Deductible
There is no deductible.
Co-insurance
No co-insurance. Specialty Dentistry: Dentist will charge 75% of the usual fee of which the employee is responsible for payment. Dental DHMO will not pay anything.
Co-payment
Co-payments vary according to service and are listed in the "Schedule of Dental Benefits" booklet.
Maximum Annual Benefit
The maximum annual benefit is unlimited.
Maximum Lifetime Benefit
The maximum lifetime benefit is unlimited.
The State of Texas Dental Choice Plan allows a choice of dentist with preventive, restorative and major services; deductibles and co-payments apply.
Premium
Premiums vary according to the level selected.
Deductible
$0 - $50 per individual per calendar year. Note: There may be separate deductibles for differing classes of services. Family limit is three times the individual deductible per calendar year.
Co-insurance
Amounts 0% to 50% depend on service classification and whether a network dentist is used.
Co-payment
Co-payments are not applicable to the Dental Choice plan.
Maximum Annual Benefit
- The maximum annual benefit $1,500 (excludes orthodontic services)
Maximum Lifetime Benefit
- The maximum lifetime benefit for orthodontic services is $1,500
- The maximum lifetime benefit is unlimited for other services.
